Sir/Madam,
With reference to the Hospital list sent to me I find that most hospitals contacts with you have expired I find that roughly you have only 32 to 33 hospitals on contract for treatment and in Mumbai only one hospital with a valid contract.
Please advice how can anybody use the benefit of the scheme with only one hospital in Mumbai on contract.
Please advice if it is possible to get treated by any hospital and get reimbursement from UTI and if so which office and office will handle such matters for reimbursement

Sir,
In case of emergency, the admission to any nearest hospital can be availed provided the doctor gives in writting that it the patient was admitted on emergency grounds. In such instances, the patient needs to settle the bills of the hospital and submit the following document to get reimbursement.
Original hospital bills along with details of treatment.
All original cash memos/ bills for medicines purchased during hospitalization.
Original Discharge card
A letter from the doctor that the treatment was given in the said hospital on an emergency basis.
5. SCUP Logbook or updated copy of the logbook.
The unitholder needs to submit the document to UTI TSL, who will get the reimbursement from NIAC and shall forward the cheque to the unitholder. The entire process takes 45-60 days.

Opp. ICICI Tower, Bandra - Kurla Complex, Bandra (E), Mumbai -- 400051
17.05.2009 contract expiry date
This is the only Hospital with a valid contract as per your list and this is a specialty hospital for Cardiac treatment only
Please advice which hospital can be used for other treatments in Mumbai as all the hospitals on list have expired contracts
Does it mean that your scheme is not valid for getting treatment other than Asian Heart Institute & Research Centre till 17th June 2009
Please revert with full detailed reply regarding all alternatives to get treatment
