I cleared XIIth boards in the year 2004 with 80% marks. This enables me to tell you about an above average student ending up with average performance i.e. myself.
I was the school topper in XIth class. Head-boy of the school in class XIIth and was targeting 90% marks. I started XIIth with zeal but it soon weaned away. I had this habit of leaving things for the eleventh hour and completed my physics study at 2A.M. on the day of the exam. I devoted disproportional amounts of time in favor of Chemistry and lacked practice in Maths.
These and other similar drawbacks are experienced by many students and that's Precisely why I am telling U all this. What I lacked (as many orhers) was sheer hard-work and regularity and was a touch over-confident. Now some advice:
Firstly- Keep Urself motivated (easier said than done). XIIth marks are a stamp on Ur forehead that U are gonna have to carry Urself with.
Secondly- Never loose faith in Urself even if U are a below avg. student. Take my example as I assure U that brains don't really matter in XII exams, what does is hard work.

Now subjectwise tips:
Don't use more than 3 books a suject . Study less books but finish them thoroughly.
PHYSICS: Complete NCERT text and Comprehensive Physics.
CHEMISTRY: NCERT text and Comprehensive/Pradeep's Chemiatry.
MATHS: R.D. Sharma
ENGLISH: NCERT(memorise little important happenings in lessons) , Any grammar book(if needed)
COMPUTER Sc.: Do as many C++ programs urself as U can and practise set-pieces like series, address calculation etc.
Be thorough with concepts and repetitive questions.
Lastly: Be confident and remember- if U study well, cleberations are just around the corner
