Sub: Claim No. HH431200524 dated 15-04-2011 not settled.
Policy No.060281/48/10/41/00000378 .
With ref. to above I have to state you that I submitted a claim of my wife Meenaxi Kantharia under no.HH431200524 to Heritage Health TPA PVT Ltd on 15-04-2011 for Rs: 24911/-. There was no response from Heritage . I enquired by my Agent and he brought the letter of "Claim not Admissable" for the query raised by them and I approached the Doctor at Shalby Hospital and she provided me a certificate that the ilness is not related to the query raised by Heritage, which was submitted on 05-08-2011 to my Agent.
Almost seven months have been passed and I have neither received a cheque nor any Reply from Heritage .
You are requested to arrange for the cheque from Heritage.
