No proper(Pre printed bill number) consultation bills provided Apollo Greams Road

All doctor consultation bills are given in cash - No card payment - avoid tax
Payment slip which was given by writing from the doctor secretary - Not valid at any insurance company for reimbursement
Mediassist is not accepting payment slip because there is no pre printed bill number in the receipt, just a slip after we give cash.
Total irresponsible secretaries to all chief doctors - do not bother about patient request or complaint. Simply acting and do not receive the phone call.
Already complained to DMS(Director of Medical Services, 2nd floor, Apollo Main block Greams Road) - Total Waste of time of money.
No action been taken till now.
I need proper scan copy of the letter head with Apollo Hospital for pre printed number on the attached items.
